Fitness Questionnaire

We start by using a questionnaire to help determine the program that best fits your goals and lifestyle. Once you submit your answer in the Contact Section below, we will evaluate the options and get you started on your fitness journey (Be sure to add "Fitness Survey" to the subject line).  Here we go!!

1) What are your fitness goals?


2) At your peak fitness level, how fit were you (1-not fit at all, 10-very athletic)


3) Do you have any recent injuries?  Describe


4) Do you have any prior injuries that limit your abilities? Describe 


5) How often do you participate in physical activity (sports, exercise, dancing, etc)


6) Do you struggle with:

  A-exercise consistency

  B-food quality

  C-portion control (too much or too little)


7) would you prefer to have a program with:

  A-Heavy to Medium weights 

  B-Medium to Light weights

  C-I don't have any weights
